Please complete the following questionnaire to the best of your ability. All entries have included in their selection a Competition Rules and Agreements document that must be read and agreed upon. Peoples Choice Judging requires specific availability detailed below! Lastly, this selection process is NON-REFUNDABLE, but IS transferrable.

Entry Agreement

This Agreement constitutes the entire understanding between the Participant and the Event Organizer, and no other promises, agreements, or representations shall be binding unless in writing and signed by both parties.

By electronically signing this Agreement, the Participant agrees to abide by all terms and conditions outlined in this contract. Failure to comply with the terms may result in disqualification from the competition.
